Ontario Health Teams (OHTs) and integrated care delivery are two key priority change initiatives for the province and Ontario Health to achieve a person-centred, sustainable, integrated health system. The West Region System Strategy, Planning, Design and Implementation (SSPDI) portfolio is seeking one (1) Lead to join our team to be a resource, subject matter expert, and planning, project, research, evaluation, and engagement leaders to advance and support OHT strategy and performance and integrated care delivery.
Reporting to the Director, System Strategy, Planning, Design and Implementation (OHTs & Home Care Modernization), the Lead, System Strategy, Planning, Design and Implementation will serve as the main point of contact for coordinating system level work within a defined cluster area, and will work collaboratively with the other team members, across the West Region of Ontario Health.
Stakeholder engagement is a key component of this role. The Lead, SSPDI (OHTs and Integrated Care Delivery) is a strong team player who places high priority on collaboration, active listening and building strong relationships. This position will be regional in nature, focusing on pertinent West Region needs, specifically (but not limited to) Ontario Health Team (OHT) strategy, performance, and support and integrated care delivery. The role requires knowledge of the unique needs of the West Region. As a result, preference will be given to candidates who live in the West Region.
Here is what you will be doing:
Provides leadership, support and expertise to assigned health system planning and integration initiatives, coordinating and conducting analysis of the local health system to identify population health needs, service gaps, and opportunities for integrated service delivery across the Ontario Health West Region.
Identifies and fosters regional relationships among health service providers and related sectors that will result in improving the health of the population, enhancing the experience of care, and sustaining integrated service delivery.
Engages local OHTs and health service providers on issues related to planning and integration across the Region; manages project plans
Provides support to OHTs on completing deliverables and reports in alignment with guidance and standards; identifies available additional supports, where needed to advance OHT maturity and performance.
Uses data to support OHTs and drives a population-based health approach to health planning
Develops and maintains relationships with OHTs for performance and development-related matters
Plays a lead role in exploring and optimizing new models of integrated care
Builds institutional knowledge in areas of specialization through presentations, writing briefing notes, preparing recommendations, contributing to reports, and leading forums for knowledge exchange, information sharing, and problem solving (including the OHT Matrix Team)
Works with West Region cross-functional team members to identify, initiate, support, monitor, and evaluate planning and integration priorities and improvement initiatives, including for home care modernization.
Participates in and contributes to provincial planning activities as well as facilitates meetings and conducts effective stakeholder consultation to plan, assess impacts of recommendations and decisions, and promote consensus among stakeholders for actions required to coordinate efforts in order to address complex system initiatives (e.g., regional delivery models, health inequities etc.)
Here is what you will need to be successful:
Education and Experience
Minimum 5 years’ experience in a multi-stakeholder environment, preferably in the health care setting
Bachelor’s degree in health administration, health planning, business administration or a related field with Master-level degree preferred; or an equivalent combination of education and experience in a health care environment
2-3 years’ experience within or supporting OHTs preferred
Experience working with senior level health leaders and other key stakeholders on complex system-level issues
Knowledge and Skills
Strong understanding of the Ontario Health Team model and the health system transformation underway in Ontario
Strong understanding of hone and community programs and services
An understanding of how health services are delivered in Ontario and a working knowledge of the role of Ontario Health in planning, managing, evaluating and funding regional health care services.
Good knowledge of and proven ability to apply multiple planning and analysis methodologies and make effective recommendations.
Understanding and experience with embedding change management and quality improvement.
Proven knowledge of best practices, evidence and tools to support integrated care, population health, health equity, and transformative change
Strong written, presentation, and verbal communication, facilitation, and interpersonal skills to work effectively with other staff and to liaise with colleagues across Ontario Health and with contacts in the Ministry.
Excellent skills in working diplomatically and collaboratively with partners with different perspectives; strong interpersonal skills and proven ability to build and maintain strategic relationships and partnerships with diverse stakeholders
Proficient in accessing the internet to support research and superior skills in the use of the MS Office suite including SharePoint, PowerPoint, Word, Excel as well as virtual collaboration/meeting platforms (MS Teams, Zoom) required
Proficiency in French is an asset
Demonstrated experience in planning process, analysis and evaluation of health care issues across multiple sectors.
Demonstrated experience engaging and working effectively in partnership with a broad range of individuals and organizations of diverse backgrounds, preferably within the health care sector.
Experience in analyzing and interpreting performance data, including financial data and quality indicators and evaluating program performance.
Uses information to make informed decisions and provide advice; exercises high degree of independence, discretion, and judgment in problem solving; demonstrates integrity and professionalism
Strong attention to detail;, excellent analytical and problem-solving skills required.
Must be highly organized and able to thrive as a team player in a fast-paced collaborative team environment while managing a demanding workload effectively in an environment of many competing priorities.
