Reporting to the Director, System Strategy, Planning, Design and Implementation, the Lead, System Strategy, Planning, Design and Implementation serves as a resource to the Integrated Care program areas and assists in the identification and implementation of appropriate planning and development methodologies. Responsible for assisting with the management of complex planning projects and initiatives to support members on the Ontario Health Central team regarding planning, project management, research, engagement activities and evaluation of programs and services.

In addition, the Lead, System Strategy, Planning, Design and Implementation is responsible for ensuring implementation of the Ontario Health Central priorities and initiatives and helps stakeholders successfully plan, execute and manage project and program management responsibilities. Mentor and lead as a subject matter expert on OHT's and homecare modernization for Central Region staff and external stakeholders.

This position will be regional in nature, focusing on pertinent Central Region needs, specifically (but not limited to) Ontario Health Team (OHT) development and support.

Here is what you will be doing:

  • Provide support to OHTs on completing funding deliverables and reports in alignment with standardized guidance and messaging

  • Identify available additional supports to help OHTs with deliverable development and performance

  • Use data to support OHTs and drive a population-based health approach to health planning

  • Develop and maintain relationships with OHTs for performance and development-related matters

  • Provide advice and input from the planning perspective to the development of health-care system funding plans and manage project plans for planning activities as they relate to key portfolios.

  • Identify emerging issues in the community that require a planning/development response and develop appropriate planning approaches

  • Work with members in the System Strategy, Planning, Design and Implementation Teams as well as the Capacity, Access and Flow team, this position will act as Ontario Health Central lead on monitoring implementation and ongoing quality improvement activities related to health-care planning.

  • Coordinate and conduct analyses of local healthcare system, based on provincial data, community input and other sources for purpose of identifying gaps and establishing planning priorities.

  • Promote high-quality research and planning and provide leadership to the development of methodologies in the profession of health planning.

  • Conduct effective stakeholder consultation and engagement related to planning, assesses impact of planning recommendations and decisions, and promotes consensus among stakeholders for action required.

  • Participate in and contribute to provincial planning activities as well as conduct effective stakeholder consultation related to planning, assessing impact of planning recommendations and decisions, and promoting consensus among stakeholders for action required.

  • Build institutional knowledge in areas of specialization through presentations, writing briefing notes, preparing recommendations and contributing to reports

  • Facilitate meetings that include representatives of partner organization and/or members of the public

  • Provide leadership for the development of a culture within Ontario Health Central that reflects the role to drive integration and coordination locally

  • Provide advice and input from the planning perspective to the development of health-care system funding plans

  • Maintain an awareness of current issues in the Ontario Health, Central Region and in the health care system with particular emphasis on integrated and coordinated health care delivery

  • Manage project plans for planning & integration initiatives

  • Understand and comply with the Occupational Health and Safety Act, Regulations, and the organization's health and safety practices, policies, and procedures

  • Contribute to a culture of patient safety, which includes respect, accountability, and open communication in a psychologically safe environment

  • Support teams to ensure health service provider and service provider organization adherence to health service accountability agreements

  • Coordinate and foster regional initiatives that improve quality and patient safety

  • Other duties as assigned

Here is what you will need to be successful:

Education and Experience

  • Bachelor's degree in health administration, health planning, business administration or a related field with Master-level degree preferred; or an equivalent combination of education and experience in a health care environment

  • Experience in project management required

  • Experience with Home and Community Care Support Services preferred

  • Proficiency with Microsoft Office tools, particularly Excel, Word and PowerPoint required

  • Bilingualism (French and English) an asset

  • Experience with Microsoft Teams and SharePoint an asset

Knowledge and Skills

  • Strong understanding of the Ontario Health Team model and the health system transformation underway in Ontario

  • Strong understanding of Home and Community Care Support Services programs and services

  • Minimum 5 years' experience in a multi-stakeholder environment, preferably in the health care setting

  • Excellent skills in working diplomatically and collaboratively with partners with different perspectives

  • Good knowledge of and proven ability to apply multiple planning and analysis methodologies and make effective recommendations using a population health management approach

  • Understanding of the current and changing dynamics of the Ontario health-care system

  • Understanding and experience with embedding quality improvement initiatives

  • Excellent communication and interpersonal skills to work effectively with other staff and to liaise with colleagues across Ontario Health and with contacts in the Ministry

  • Must be highly organized and able to thrive in a face paced environment with multiple priorities

  • Uses information to make informed decisions and provides advice; exercises high degree of independence, discretion and professional judgment in problem solving

  • Strong written and verbal communication skills. Internally, communicates with colleagues throughout the organization to present, discuss information and problems related to financial management and funding

  • Externally, communicates directly with various stakeholders to monitor funding allocations and performance

  • Strong attention to detail, demonstrated integrity and professionalism with a solid understanding of financial controls, financial reports and budget management

  • Excellent analytical and problem solving skills required

  • Must be highly organized and able to thrive in a fast-paced environment with multiple priorities.
