Description

This position will provide out-reach services to clients living in the community using a wrap-around approach to service.

This position will support clients in developing skills for living independently (based on the client needs) and assist clients in finding permanent or long- term housing in the community.

KEY RESPONSIBILITIES:

  • Provide case management services to clients
  • Oversee clients transitioning into the community
  • Record accurate information in client case notes regarding services provided as well as observed behaviour of clients and general assessment of interaction with clients
  • Work collaboratively with clients and Caseworkers to identify needs, goals, resources, and actions required to meet identified targets for clients
  • Respond to client's issues and concerns appropriately as necessary
  • Make referrals to community agencies as requested/required by clients; and liaise with other service providers collaboratively to provide interagency case management, especially when dealing with individuals with very complex needs
  • Provide clients with necessary information and support in navigating different systems and social service connections needed to obtain vital resources that assist in addressing underlying factors contributing to individual(s)' experience of homelessness (ex: identification, financial support, employment assistance, counselling, education, treatment etc.)
  • Support programming staff in the delivery of program opportunities for clients, including programs, promotion and information to clients; facilitation support; and program evaluation/ feedback

Perform other duties as required.

WORKING CONDITIONS:

  • This is a permanent full-time position
  • Flexibility in scheduling is required. The work schedule is determined in consultation with your supervisor and may vary according to requirements of responsibility

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

  • Completion of a formal post-secondary/college program of two academic years.
  • Alternative combinations of education and experience may be considered

EXPERIENCE AND KNOWLEDGE:

  • Minimum of two (2) years of prior related experience, including relevant experience
  • Crisis management and de-escalation training is an asset.

SKILLS AND CAPABILITIES:

  • Attention to detail, problem solving and analytical skills.
  • Strong sense of integrity and confidentiality with professional ethics and a balanced sense of fairness and flexibility.
  • Possesses strong risk assessment skills as it pertains to client care
  • Strong oral and written communication skills and effective interpersonal skills
  • Ability to work both independently and as part of a team
  • Enhanced Reliability Screening required
  • Ability to undergo applicable screening and background checks successfully that are satisfactory to The Salvation Army, in its sole discretion (i.e., The Salvation Army Abuse Registry) and provide an original copy of a Background Check with vulnerable sector screening
  • Ability and willingness to develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities
